ABSTRACT

Objetivo: compreender os fatores que interferem no diagnóstico da sífilis em homens à luz da Teoria da Diversidade e Universalidade do Cuidado Cultural. Método: estudo qualitativo, aprovado por Comitê de Ética em Pesquisa, realizado em Hospital Universitário no Rio de Janeiro, entre setembro de 2017 e março de 2018. Foram entrevistados 32 homens diagnosticados com sífilis adquirida, e a análise temática foi utilizada para o tratamento dos dados. Resultados: a maioria dos homens tinha história de contágio por outra Infecção Sexualmente Transmissível e foi diagnosticada no ambulatório de imunologia. O momento diagnóstico é encarado com surpresa, tem repercussões psicossociais e é influenciado por fatores culturais e sociais. Considerações finais: alguns fatores interferem positivamente e outros negativamente no diagnóstico da sífilis na população masculina. Para detectar essa infecção nos homens, deve-se conhecer o contexto sociocultural em que estão inseridos para, assim, implementar estratégias tanto diagnósticas quanto preventivas mais eficazes.


Objective: to understand the factors that interfere with the diagnosis of syphilis in men in the light of the Theory of Diversity and Universality of Cultural Care. Method: qualitative study, approved by the Ethics and Research Committee, conducted at a University Hospital in Rio de Janeiro, during september 2017 to march 2018. Thirty-two men diagnosed with acquired syphilis were interviewed, and thematic analysis was used for data treatment. Results: most men had a history of contagion by another STI and were diagnosed at the immunology outpatient clinic. The moment of diagnosis is faced with surprise, has psychosocial repercussions and is influenced by cultural and social factors. Final considerations: some factors interfere positively and others negatively in the diagnosis of syphilis in the male population. To detect this infection in men, it is necessary to know the sociocultural context in which they live, in order to implement more effective diagnostic and preventive strategies.

Método: estudio cualitativo, aprobado por el Comité de Ética en Investigación, realizado en un Hospital Universitario de Río de Janeiro, de septiembre de 2017 a marzo de 2018. Se entrevistó a 32 hombres diagnosticados de sífilis adquirida y se utilizó el análisis temático para el tratamiento de los datos. Resultados: la mayoría de los hombres tenía antecedentes de contagio por otra Infección Sexualmente Transmisible y el diagnóstico se hizo en el ambulatorio de inmunología. El momento del diagnóstico se afronta con sorpresa, tiene repercusiones psicosociales y está influenciado por factores culturales y sociales. Consideraciones finales: algunos factores interfieren positivamente y otros negativamente en el diagnóstico de la sífilis en la población masculina. Para detectar esta infección en los hombres, se debe conocer el contexto sociocultural en el que se insertan, para entonces poner en marcha estrategias de diagnóstico y de prevención más eficaces.

Resumen | Introducción. La sífilis gestacional se considera de interés en salud pública por las repercusiones que tiene en la madre y el hijo. Pese a tener protocolos para su notificación, diagnóstico y manejo, en Colombia se siguen evidenciando limitaciones en su control. Objetivo. Describir las características sociodemográficas, clínicas y de distribución espacial de las pacientes con sífilis gestacional en Cali, Colombia, en el 2018. Materiales y métodos. Se hizo un estudio transversal con 427 casos de sífilis gestacional reportados al Sistema de Vigilancia en Salud Pública (Sivigila). Para el procesamiento estadístico, se utilizó el programa R, versión 3.5.3. Las variables cualitativas se presentan como proporciones y, las cuantitativas, mediante medidas de tendencia central y dispersión, Para la distribución espacial, se usó el programa Qgis 3.0. Resultados. La razón de sífilis gestacional fue de 17 casos por 1.000 vivos (incluidos los mortinatos). El 57,1 % de las pacientes pertenecía al régimen subsidiado de salud y el 16,6 % no estaba asegurado. El 90,4 % de los casos se diagnosticó durante el embarazo; el 47,2% recibió tres dosis de penicilina y el 57,6 % de los contactos recibió tratamiento. Conclusiones. La tasa de sífilis gestacional en Cali en el 2018 fue superior a la nacional y la enfermedad se presentó con mayor frecuencia en mujeres gestantes en condición de vulnerabilidad socioeconómica, lo que coincidió con la distribución espacial en general. Se evidenció la falta de oportunidad en la detección temprana y el manejo de la infección tanto en las pacientes como en sus contactos, lo cual dificulta el control de la enfermedad y refleja la inadecuada aplicación de la ruta integral de atención en salud materno-perinatal.


Abstract | Introduction: Gestational syphilis is considered an event of public health interest given its impact on mother and child. In Colombia, despite having specific protocols for its notification, diagnosis, and management, there are still limitations in its control. Objectives: To describe the sociodemographic and clinical characteristics, as well as the spatial distribution of gestational syphilis in Cali, Colombia, in 2018. Materials and methods: We conducted a cross-sectional study of 427 gestational syphilis cases reported to the Colombian national epidemiological surveillance system (Sivigila). For the statistical processing, we used the R program, version 3.5.3. We expressed qualitative variables as proportions and quantitative ones through central tendency and dispersion measures, and to establish the spatial distribution we used the Qgis program, version 3.0. Results: The prevalence of gestational syphilis was 17 cases per 1,000 live births (including stillbirths); 57.1% of patients belonged to the subsidized healthcare system and 16.6% had no health insurance; 90.4% of cases were diagnosed during pregnancy; 47.2% of the pregnant women received three doses of penicillin, yet only 57.6% of contacts were treated. Conclusions: The prevalence of gestational syphilis in Cali during 2018 exceeded the national rate with a higher frequency among women in socioeconomic vulnerability conditions consistent with the general spatial distribution. There was a lack of opportunity in the early detection and management of the infection both among pregnant women and their contacts, which hinders the control of the disease and reflects the inadequate application of the comprehensive maternal and perinatal health care route guidelines.

ABSTRACT

Objective To analyze the syphilis epidemic characteristics and trends of Yongchuan District during 2010? 2015 to provide a basis for formulating the prevention and control strategy of syphilis.Methods The data of syphilis cases reported in Yongchuan District of Chongqing City during 2010-2015 were arranged,checked anal statisticallyanalyzed.Results The incidence rate of syphilis showed a reverse U type change trend from rising to declining,latent syphilis accounted for the highest proportion,congenital syphilis cases were decreased year by year.The analysis results showed that the constituent ratio of latent syphilis and congenital syphilis had statistical difference between 2010-2013 and 2014-2015(P<0.05);syphilis high prevalent area was concentrated at the city and town area,while prevalence rate was slower in rural area.Most of syphilis patients were concentrated in the age group of 25-45 years old.There was no difference between women and men;syphilis cases generally had lower educational level,majority concentrated at the population of junior high school or below;the occupations in the syphilis patients were concentrated at the entertainment personnel,followed by migrant workers.Conclusion The incidence rate of syphilis is changed from rising to smoothly and steady decreased.It is necessary to adopt the pertinent prevention and control measures for the high-risk population.
